Primary Purpose of Care Worker
As a Community-Based Care Worker , you will play a crucial role in supporting individuals within the RAW community by assessing their needs and developing care plans, while coordinating and connecting with services and community resources to enhance their overall well-being. RAW's Care Worker will work in collaboration with other RAW staff, will be supervised and supported by the Clinical Director, and will support specific youth who attend RAW's weekly programming. To best serve individual youth, developing trusting relationships with them, their caregivers, RAW staff, and the larger community is a key component of this position. Using a trauma-informed and strengths-based model, Raw seeks to meet youth where they are in their journey to adulthood and create supportive spaces with safe boundaries for youth to be seen and heard. We seek a Care Worker who can offer practical guidance and support with finding and accessing vital resources outside of the RAW building to meet individual and family needs. This position requires a compassionate, flexible, patient, persistent, and organized professional who can collaborate with teams of people to provide comprehensive, holistic care to participants assigned to them. The day-to-day work is similar to that of a traditional case worker but within the setting of a community arts program rooted in art therapy.

Essential Functions:
· Conduct thorough interview assessments of specific RAW participants to identify
their strengths, needs, and goals.
· Develop individualized care plans and action steps in collaboration with participating youth and their caregivers, considering their unique circumstances and preferences.
· Collaborate with RAW staff who work with youth in your caseload to support individual goals.
· Collaborate with external agencies, organizations, and service providers to address youth needs, overcome barriers to access available support, and enhance the continuum of care.
· Builds strong relationships with local community resources and partners with reputable organizations and practitioners.
· Provide crisis intervention and support to participating youth who are facing immediate challenges.
· Develop crisis management plans to mitigate risks and ensure the safety of the youth.
· Regularly assess and evaluate the effectiveness of interventions and adjust care plans and therapeutic methods accordingly.
· Intermittently attend RAW events, visual art groups, film groups, and college and career access groups to support youth with higher needs and may struggle in a larger group setting. Youth who are neurodivergent, have trauma histories, have heightened anxiety, depression, or symptoms of other mental illness or disability, may occasionally need additional support from the Care Worker while in a RAW group.

Non-Essential Functions:
· Continually improves competence by attending supervision and staff meetings, accessing educational resources, and engaging in professional development opportunities while balancing work requirements with learning opportunities.
· Complies with proper paperwork procedures, prompt follow-up communications and departmental policies.
Requirements and Qualifications:
· Bachelor's in social work, psychology, a related field, or equivalent experience.
· Minimum 2 years of direct work with youth or families, preferably in case management.
· Exceptional communication and organizational skills required.
· High level of emotional intelligence (self-awareness, empathy, and social skills).
· Fluency in both English and Spanish required.
· A Massachusetts driver's license is preferred.
Physical Requirements:
This is a partially physical job by nature. Requires visiting local service providers and organizations that offer community resources, being able to sit at a desk and use a computer and phone for most of the day, bending and lifting items less than 20 pounds as needed.
Internal and External Contacts:
· Interfaces regularly with the general employee population in person.
· Interfaces regularly with members in person and on the phone.
· Interfaces regularly with outside vendors in person and on the phone.
Reporting Structure:
This position reports to the Clinical Director
