Older Adult Program Social Worker

Job description
PRIMARY RESPONSIBILITIES:
- Help older adults establish connection to virtual activities, tele-health appointments and interpersonal activities, supporting participants in avoiding isolation and to age in place.
- Conduct assessments with older adults in the community.
- Collect and enter data.
- Provide outreach to isolated older adults.
- Assist older adults with technology.
- Provide individual counseling in older adults’ homes.
- Consult with older adults’ families.
- Provide psychoeducation and coaching to older adults.
- Perform other tasks as needed.
EDUCATION, EXPERIENCE, AND REQUIRED
● LMSW preferred.
● Knowledge of a variety of operating systems i.e., Apple, Android, Microsoft.
● Experience working with older adults strongly preferred.
● Ability to travel throughout Queens county.
● Comfortable working within participants' homes.
● Strong advocacy and creative problem solving skills.

SALARY RANGE $65K-$75K annually
