Key Responsibilities:
As a Behavioral Health Consultant, you will:
Provide assessment, consultation, and brief interventions to clients and families in a supportive and collaborative manner.
Work seamlessly within a multi-disciplinary team to address psychological or behavioral concerns, advocating for patients when necessary.
Create and maintain a patient-centered environment through effective communication and positive interactions.
Support organizational policies, compliance with regulatory agencies, and contribute to the strategic plan of the organization.
Essential Job Functions:
Offer brief consultation-based services to patients and providers within an integrated care model.
Identify and assess patient concerns swiftly, developing meaningful behavioral-based interventions.
Assist clients in achieving mental health and lifestyle goals, including addressing factors such as tobacco use, substance abuse, nutrition, and physical activity.
Evaluate and apply appropriate interventions in crisis situations.
Collaborate with the primary care team to enhance self-management of chronic diseases through processes, behavioral interventions, and patient education.
Assist in the detection of "at-risk" patients and develop plans to prevent further psychological or physical deterioration.
Collaborate with various departments to implement crisis management procedures and referral processes.
Facilitate group meetings and provide communication skills training when needed.
Foster an inclusive and safe environment through an understanding of cultural humility during interactions.
Job Qualifications:
Required:
Master's degree in counseling, social work, psychology, or related behavioral health sciences.
Licensed Clinical Social Worker (LCSW) or Licensed Professional Clinical Counselor (LPCC) licensure.
Working knowledge of SBIRT, motivational interviewing, and other evidence-based interventions.
Ability to work within the pace of a primary care setting and in an interdisciplinary team.
Belief in trauma-informed care and harm reduction principles.
Preferred:
Experience in a Primary Care Behavioral Health Model.
Prior experience working with diverse population groups.
Knowledge of local resources and supports.
Experience and/or knowledge of evidence-based treatment for substance use disorders.
Working Conditions:
Work is performed in a clinical setting with some hands-on patient care.
OSHA category: High risk, involving exposure to blood, body fluids, or tissue.
Physical Requirements:
Primarily a desk job with occasional lifting or carrying items appropriate for an office environment, not exceeding 20lbs.
