Heart Disease Program Specialist (4702032) Location Jefferson City, MO :

Job Location: This position will be located at 930 Wildwood Drive, Jefferson City, MO 65109

In this position, you will promote and support heart disease prevention and management efforts across the state. You will build relationships and provide technical assistance to multiple partners and networks. Most importantly, you will be supporting individuals and communities in living healthier lives and reducing heart disease rates and health disparities.

• Develop and foster relationships with stakeholders to advance cardiovascular disease initiatives across the state
• Coordinate and facilitate the Missouri Cardiovascular Health Learning Collaborative to achieve grant deliverables
• Coordinate and implement projects to improve cardiovascular health
• Partner with health professionals, local health administrators, contractors, and other organizations to create clinic-to-community linkages to increase referrals to lifestyle change programs.
• Create, facilitate, and monitor letters of agreement and contracts with partners to achieve program goals.
• Contribute to required grant reports and updates.
• Travel will be required with possible overnight stays and overtime.

• Bachelor's degree and 1-3 years of relevant experience (Substitutions allowed.)
• Excellent relationship-building skills
• Strong time management skills
• Program management experience
