Job description

The case manager's major function is to direct and coordinate the integration of Case Management functions into the patient care, discharge, and home planning process with other hospital departments, external service organizations, agencies, and health care facilities. In addition, the case manager serves as the clinical resource for progressing care, discharge planning, and transitions of care.

At ContinueCARE Hospital, we take pride in the care we provide for patients with an average length of stay of 25 days in a variety of specialty programs. Our staff enjoy our small-hospital environment inside of a large acute care hospital.

Qualifications:
Graduate of an Accredited Nursing Program.
Unrestricted RN license.
3 - 5 years of Clinical Nursing Experience Required.
Clinical Documentation Improvement Experience Required.
Previous Case Management Experience Preferred.
BLS required.
