Job description
East Bay Community Action Program (EBCAP) is seeking a Per Diem (1.5-2 hours weekly) Dietitian / Nutritionist for our Office of Volunteer Services Department located at 100 Bullocks Point Avenue in East Providence, RI. We are seeking a motivated individual who is committed to enabling elders to live independently and comfortably in the community.
Counsel clients on nutrition issues and healthy eating habits. Help prevent dietary health conditions by providing nutrition counseling to the elderly. Educate clients about food nutrition and how it affects the quality of life and health care issues. Contact clients, assess their current nutritional habits and comprehension, and help them understand how healthy eating and diet play roles in their overall health. In some cases, you will provide more information for your clients, and other times work with the client to develop health and nutrient goals and a new meal plan that contributes to the patients reaching their goals.
- Follow up with Congregate Nutrition Program clients identified as nutritionally at risk on the Personal Information client form.
- Conduct appropriate screenings and/or assessments to provide seniors and differently-abled individuals with resources and wrap-around services.
- Identify barriers to improve nutrition outcomes, and provide information to achieve better health and quality of life.
- Guide individuals to practice good nutrition and reinforce healthy nutrition goals.
- Create educational materials about healthy food choices and lifestyle.
- Weekly reporting of PIFs to the CNP Coordinator and provide outcomes
MINIMUM REQUIRED EDUCATION A licensed Dietitian / Nutritionist or other person with equivalent education and training in nutrition science, or if such an individual is not available an individual with comparable expertise in the planning of nutritional services.
