QUALIFICATIONS:
* Bachelor of Science in Nursing (BSN) preferred or Associate Degree in Nursing (ASN).
* Current Indiana Registered Nurse license.
* 3 years of experience working as a Registered Nurse.
* Current CPR certification.
* Insurability by the agency's liability insurance carrier.
* Strong oral and written communication and mathematics computational skills are required.
Specific skills needed:
* Basic knowledge of Microsoft Word, Excel, PowerPoint, Outlook, Google
Knowledge of:
* Principles, methods, techniques, strategies, and trends in educational health services;
* Applicable health appraisal methods, techniques, and procedures;
* Social service and health service agencies in the local area;
* Social, emotional, and behavioral characteristics of student clients;
* Health services program evaluation and research techniques, strategies, and procedures;
* Appropriate curriculum and instructional programs pertaining to student health education;
Ability to:
* Assist students in effectively dealing with health issues, problems, and concerns;
* Effectively work with site and District personnel, parents, other health care professionals, physicians, social service and youth service agencies in resolving student health problems and concerns;
* Effectively participate in the planning and implementation of health instructional processes;
* Communicate effectively in oral, written and electronic formats, and as required by the assignment of utilizing a second language, if bilingual and if needed and applicable;
* Understand and carry out oral and written directions with accountability controls;
* Establish and maintain effective organizational, public, and community relationships.

REPORTS TO: Building Principal
JOB GOAL: Under direction, to provide basic health services to students; to assist in the planning and implementation of school health programs in cooperation with medical authority and school administrative personnel; to provide health and wellness educational programs for students and staff; to evaluate the health status of students and the health service program; to provide instruction in health practices, first aid, child care, and other related subjects; to serve as a resource pertaining to student health management strategies and welfare and attendance problems and concerns; and to do other related functions as required.
ESSENTIAL FUNCTIONS: This position classification requires subject matter expertise commonly associated with school and public health services knowledge and skills. Responsible and directly related nursing and health provider experience is required to perform problem analysis using widely prescribed and used principles and concepts. The position classification performs decision analysis and makes decisions of considerable consequence, requiring the application and interpretation of data, facts, procedures, and policies. The incumbents meet with school administrators, teachers, internal staff and members of the community to communicate information, provide data, and assist with problem solving. This position may require walking and standing for extended periods. This position may require moving and transporting patients which may involve heavy lifting. This position requires accurate perception of sound, near and far and color vision, depth perception, handling and working with materials and objects, and providing oral information.

PERFORMANCE DUTIES AND RESPONSIBILITIES:
* Provides basic medical care and first aid to the students, staff and visitors of the Hammond Academy of Science and Technology in accordance with all policies, procedures and guidelines.
* Maintains accurate and up-to-date medical records on all students, including CHIRP information.
* Attends and participates in building and department staff meetings.
* Attends and participates in case conferences and writes Individualized Healthcare Plans (IHP) for students with severe medical needs and all diabetic students.
* Attends and participates in 504 conferences within the building.
* Maintains infection control in the building by performing a variety functions and activities pertaining to communicable disease prevention and control.
* Assists in writing and implementing policies for exclusion and readmission of students with contagious, infectious, and/or communicable diseases.
* Collects, organizes, and analyzes student health information, and provides teachers and school administrative personnel with student medical conditions on a need to know basis while maintaining strict confidentiality.
* Provides information and guidance to students and staff pertaining to individual student mental and physical health needs as determined by health appraisals and standards of growth and development.
* Coordinates the school state-mandated student immunization program, including parent education and referral programs.
* Provides information and data to students concerning health education, nutrition, diet, and social and communicable disease prevention and treatment.
* Serves as a health expert and resource to site and District personnel, and members of the school community.
* Develops and participates in Wellness programs.
* Participates in building and Health Services staff meetings.
* Meet parents, school personnel, and members of various youth serving agencies in developing alternative solutions to student health issues, problems and concerns.
* Serves as a liaison between social service and health service agencies in pursuing health referral follow-ups.
* Collects data to determine health services program effectiveness.
* Assists in the planning, development, and implementation of procedures to provide for health service emergencies.
* Assists with on the job injury prevention and reporting.
* Assists in identifying health service program needs, and participates in developing school health instructional programs.
* Establishes, maintains, and monitors the confidential medical records pertaining to student services and related matters.
* Assists in dealing with health related student behavior management problems.
* Plans, develops, and presents, as requested, a variety of management related reports pertaining to the health service functions and activities.
* Performs all state-mandated vision, color vision and hearing screenings and makes referrals as needed.
* Completes state-required reports on immunization, vision, and hearing screenings.
* Maintains all student records, notes, and logs in Skyward.
* Provides training to staff and parents regarding the medical needs of children.
* Provides Blood Borne Pathogen/Universal Precautions training to all staff each year.
* Provides instruction to students on dental health and puberty (elementary nurse).
* Provides instruction to high school students on Breast Self-Exam and Testicular Self-Exam.
* Maintains medical supplies and equipment in health office.
* Participates on various committees within the school district as requested.
* Assists as a member of the crisis intervention team and building Safety Committee.
* Assists in the identification of "at-risk" children.
* Performs all other duties as directed by the building principal or designee, the Coordinator of Health Services and/or Central Administration.
Salary: $27.00 - $35.00 per hour
