Bilingual Spanish - Medical Records Coordinator (FT)
Summary: Provides support for medical record retrieval, data input, and other duties related to patient visit documentation and medical records.
Description of Duties and Responsibilities:
- Accurately inputs all findings to and from the medical record.
- Retrieves medical records for patient care and/or visit monitoring.
- Reports training needs.
- Identifies and reports barriers to department effectiveness.
- Fosters positive interpersonal relationships with other staff members. Practices effective teamwork.
- Provides accurate verbal communication to appropriate persons in a clear, concise manner.
- Utilizes time and supplies efficiently.
- Maintains confidential information regarding the patient and work-related matters.
- Maintains/ Process Patients’ records in an efficient and timely manner.
- Handle patient information with utmost respect and confidentiality.
- Support the organization’s mission, vision, and values of quality care and patient satisfaction.
- Other duties as assigned.
Schedule of Essential Duties:
- Scan and index medical records received into EPIC Care.
- Distribute incoming e-faxes every morning and afternoon according to the task schedule.
- Accept incoming documents via mail from ChapCare internal courier.
- Fax and mail requested the medical release of records. Scan requests and Release Information into a chart (in EPIC Care.)
- Take all medical record phone calls and triage calls.
- Complete Social Security release of records and log in EPIC Care for tracking.
- Copy and send a release of records for attorney’s offices and log in EPIC Care for tracking.
- Receive and triage Subpoena’s for release of medical records daily and log in EPIC Care for tracking.
- Other duties as assigned.
Qualifications for Position:
- Three (3) years of experience in the field related to medical or health care, one (1) year experience of which is directly related to the medical records process (preferred but not required). Education or Experience may be substituted. Experience with EHR preferred (not required).
- Associate degree from an accredited university.
- CPR Certified.
Skills and Knowledge Required:
- Computer data entry experience.
- Word processing experience.
- Ability to retrieve medical records in an electronic database as an Electronic Medical Record.
- HIPAA Rules and Regulations.
- Bilingual Spanish.
Prior Experience:
- In Medical Records for 3-5 years preferred, but not required.

New health center locations are now open in South El Monte, El Monte, and Monrovia. ChapCare also offers free health insurance enrollment assistance through a Covered California Navigator Grant. Our Certified Enrollment Counselors assist community members enroll in health insurance plans available under the Affordable Care Act (ACA) at select health centers in Pasadena, El Monte, and Monrovia. During the COVID-19 Pandemic, ChapCare has answered the call as a public health resource for our community. Many of our healthcare services can now be delivered through telehealth options such as telephone visits. In addition, ChapCare offers COVID-19 testing options for its patients and the community. ChapCare also regularly sanitizes its facilities following all safety guidelines set forth by CDC, state, and local health officials.
