POSITION DESCRIPTION:

Provides medical case management to Department of Children and Family Services (DCFS) wards in assigned counties for the purpose of facilitating access to medical care, coordination of services, conducting follow-up.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
  1. Complete assessment of clients’ health and social service needs.
  1. Develop an Individualized Care Plan for each client based on needs assessment and recommendations from primary care provider and service providers.
  1. Initiate referrals to appropriate healthcare and service providers.
  1. Maintain accurate documentation and up to date case files according to program protocol.
  1. Provide education to foster parents and case workers.
  1. Conduct outreach to healthcare providers
  1. Communicate with medical providers, DCFS, DHS, and MCO staff.
  1. Maintain client confidentiality.
  1. Complete required training.
  1. Perform other duties as assigned.
KNOWLEDGE, SKILLS, ABILITIES:
  1. Ability to work independently with minimal supervision.
  2. Ability to prioritize and multitask.
  3. Ability to communicate effectively and professionally.
  4. Ability to maintain confidentiality of client and program information.
  5. Strong attention to detail and organization skills.
  6. Proficient computer skills: Microsoft Office Word, Excel, and electronic health records.
EDUCATION AND EXPERIENCE:
  1. A registered professional nurse licensed by the state of Illinois and two years of experience in community health or maternal child health nursing. OR
  1. A Bachelor of Science in Nursing (BSN) degree from an accredited program and one year of experience in community health or maternal child health nursing.

PHYSICAL DEMANDS:

Light physical activity performing non-strenuous daily activities of an administrative nature.

WORK SCHEDULE:

Normal work hours for this position are Monday through Friday dayshift; 8:00a.m. To 5:00p.m.
