Job Description
The Clinical Care Team Associate will assist nursing staff in providing direct patient care from the admission process to activities of daily living through patient transfer and discharge.
- Assist nursing staff /ancillary staff in transferring patients from bed to bed, stretcher, or wheelchair, picking up and delivering patient meal trays, and assisting with patient feeding as directed.
- Demonstrate the ability to organize workload and manage multiple tasks while maintaining a clutter-free and orderly environment, including prompt answering of unit telephones and accurate delivery of messages and information to patients, families, visitors, staff, and personnel upon request.
- Educate and train new staff members in the responsibilities of the Care Team Associate role. Perform other duties delegated by nursing staff and unit management in routine and emergent situations.
- Perform patient position changes, placement, and removal of patient equipment, daily hygiene care, simple non-sterile dressing changes, finger stick glucose checks, may perform venipuncture to obtain a blood sample, maintain quality control checks, and deliver/retrieve lab specimens and supplies as requested.
- Responsible for accurate and timely entry of information into the bed-management system.
- Responsible for prompt response to patient call lights and addressing patient needs using positive guest relations skills while ensuring patient vital signs, intake, output, height, and weight are obtained and documented as directed.
- May complete unit/department-specific duties as outlined in department documents.
Required Qualifications
One of the following:
- Completion of a Fundamentals of Nursing course within 180 days; or
- Certified Nursing Assistant (CNA); or
- Experience with direct patient care such as a medical assistant, nursing assistant, emergency medical technician (EMT), or surgical technician in the past three (3) years; or
- Completion within the last three (3) years, or will complete within 180 days, an Unlicensed Assistive Personnel (UAP) training program; or
- Nursing licensure candidate who was unsuccessful on the state licensure examinations in the past three (3) years; or
- Completion of a nursing program outside the United States and awaiting the licensure examination in this country.
- Completion of an above qualifications pathway, including patient care experience or a supervised clinical log, within 180 days as a condition of continued employment in this job classification.
