Work Life Balance **$15,000 Sign-On Bonus** for Full Time and $7,500 Sign-On Bonus for Part Time.
Summary of position/Objective: To The Rescue is a growing Home Health Care company that is seeking a Full-Time Registered Nurse (RN) Case Manager to coordinate and provide compassionate skilled nursing care on an intermittent basis for adult patients in their homes. This position is Monday-Friday with occasional weekends.
The Case Managing Nurse is part of a healthcare team (patient, family, nurse, physician, therapist and others as applicable) that work together to help our patients reach and maintain their highest level of independence with the goal to remain safe at home. Along with other duties, the nurse will perform direct care skilled visits according to the physician plan of care orders, complete comprehensive patient assessments, develop a plan of care in coordination with the healthcare team and communicate effectively to meet the patients needs. The nurse will ensure that services are delivered according to licensing guidelines, professional medical standards and agency policies and procedures.
Essential Duties and Responsibilities: - Provides services in accordance with the plan of care
- Makes the initial evaluation visit and regularly evaluates the patient's nursing needs
- Initiates the plan of care and necessary revisions
- Furnishes those services requiring substantial specialized nursing skills
- Initiates appropriate preventive and rehabilitative nursing procedures
- Prepares required documents/ notes for patient each day and submits to the school district as required
- Coordinates services
- Informs physician and other personnel of changes in the patient’s condition and needs
- Participates in in-services programs, supervising and teaching other nursing personnel
- Understands and adheres to established Agency policies and procedures
- Gives total patient care as needed
- Observes confidentiality, safeguards all patient related information
- Attends staff meetings and patient care conferences as scheduled
- Completes documentation and paperwork in a timely manner per school district/ agency policy
- Immediately reports to Clinical Manager any patient incidents/ variances or complaints
- Demonstrates competent performance of technical skills according to established procedures
- Understands and adheres to established policies/procedures
- Adheres to Agency standards and consistently interprets and accurately performs all assigned responsibilities
- Maintains acceptable attendance status per Agency policy
- Reports all incomplete work assignments to Clinical Manager
- Demonstrates sound judgement, critical thinking and decision making
- Maintains current CPR certification
- Performs other duties as assigned
Competencies/Qualifications/Education: - Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing
- One (1) year nursing experience preferred
- Acceptance of philosophy and goals of this Agency
- Ability to exercise initiative
- Ability to apply principles of logical or scientific thinking to a wide range of intellectual and practical problems
- Have knowledge of auditing and monitoring procedures
- Have working knowledge of applicable laws and regulations affecting Iowa and Colorado
- Have working knowledge of privacy laws and regulations
- Must possess strong leadership skills
- Must have excellent communication skills and the ability to communicate effectively with staff and provide instruction and direction to employees on maintaining compliance with policies, rules and procedures
- Proficient with Microsoft Office (Word, Excel, Outlook) and the ability to learn computer programs for scheduling and timekeeping
Certificates/Licenses/Registration: - Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing
- One (1) year nursing experience preferred
- The employee must provide and maintain a valid driver's license the entire duration of their employment, and must be eligible for the agency’s vehicle insurance.
Supervisory Responsibility: This role will not directly supervise employees.
Work Hours: Hours are varied depending on the needs of the organization.
Part-time and Full-time positions available
Work Environment: The work environment is variable, anywhere from office setting to residential community living.

Travel: Frequent travel within a 25-mile radius is expected for this position. Occasionally travel beyond a 25-mile radius may be required, including out of state travel.
