Job description
This is a part time position that will work Friday, Saturday, and Sunday with autonomy and flexibility to work between the home setting and the office setting.
Requires valid Arkansas RN license without restriction, current BLS/CPR certification. Hospice knowledge and experience required.
General Description: The Intake Specialist is an integral part of the Interdisciplinary Team and works closely with the hospice physicians, patients and their families. Primary responsibility is the coordination and execution of the admission process into Circle of Life Hospice care. Develops and manages initial contact with hospital discharge planners, nursing homes and assistive living facilities, patients, families or responsible parties, and clinic personnel to initiate the hospice referral. Collects data in regard to the referral process, then accurately documents data into the EMR. Assists with the admission process, including all mandatory admission paperwork and its timely and accurate completion. Facilitates the timely conversion of referral-to-admission through coordination with team members and other departments as needed. Communicates the Hospice philosophy and concept to patients and families effectively. Schedules admission visits and maintains the often-fluid schedule. Deals with human suffering compassionately while maintaining professional boundaries. Has excellent clinical skills and is able to identify problems, consult with the physician or other team members for solutions to meet patients' needs. Must have knowledge of hospice regulatory guidelines, eligibility criteria, and general knowledge about insurance coverage.
