Job description
The Substance Abuse Specialist is a licensed professional that is an integral part of the ACT team of behavioral health clinicians. The Substance Abuse Specialist will be responsible for:
- leading the ACT team in clinical meetings regarding substance abuse issues and incorporate findings into service plans
- leading the ACT team's integrated dual disorder treatment (IDDT)
- identifying ACT services clients should receive based on severity of substance use diagnosis, stage of change and immediate needs and recommend treatment objectives for the client's service plan
- providing substance use assessment and substance use diagnosis, promote harm reduction and apply assertive outreach and stage-wise treatment
- engaging in assessments to determine the degree risk for substance use and associated harmful behaviors, and the types of interventions required to work towards recovery
- making a determining when a client's severity of substance abuse may require Inpatient Rehabilitation, Medically Managed Detoxification, Chemical Dependence Medically Supervised Inpatient and Outpatient Withdrawal, and or other OASAS licensed programs and harm reduction.
- making referrals to and acting as liaison and advocate for clients who require the aforementioned external substance use interventions.
Must have at least 3 years of experience working in human services with an SMI/SUD population and extensive knowledge of Assertive Community Treatment, community integrated treatment, support services, and resources.
