Job Summary
As the Home Infusion Intake Coordinator at AmeriPharma's Specialty Care IV division, you will play a vital role in ensuring patients receive seamless and high-quality home infusion therapy services. This position involves collaborating with healthcare professionals, patients, and insurance providers to facilitate the intake process for home infusion therapy. Your dedication to patient care and coordination skills will be essential in making this transition as smooth as possible.
Duties and Responsibilities
- Patient Assessment: Conduct comprehensive assessments of patients to determine their eligibility and
suitability for home infusion therapy. This includes evaluating their overall medical condition, social and living
situation, and identifying any potential barriers to successful home infusion. - Insurance Verification: Collaborate closely with our billing department to verify insurance coverage
and benefits related to home infusion therapy. - Coordination: Work in close conjunction with healthcare providers, including physicians, nurses, pharmacists,
and discharge planners, to coordinate the transition of patients from hospital or clinic-based care to home
infusion therapy. Communicate the specific needs and treatment plans of patients to the relevant professionals. - Patient Education: Provide thorough and clear information to patients and their families about the home
infusion therapy process, the transition of care, and the procedure for medication delivery. - Documentation and Reporting: Maintain accurate and up-to-date electronic records of patient information and
track their progress throughout the intake process. - Quality Assurance: Ensure strict adherence to all applicable healthcare regulations, standards, and
organizational policies. Implement quality assurance measures to monitor patient satisfaction, address any
issues promptly, and continuously improve the intake process. - Collaboration: Foster strong working relationships with the pharmacy and clinical teams to facilitate the
accurate ordering and timely delivery of medications, supplies, and equipment required for home infusion
therapy. - Scheduling: Manage patient refill schedules efficiently, maintaining an organized and up-to-date refill calendar
to ensure uninterrupted care.
Required Qualifications
- Knowledge of home infusions therapy (specifically TPN therapy), including various administration methods and associated equipment is a plus.
- Excellent communication skills, both written and verbal, to effectively interact with healthcare professionals, patients, and third-party vendors.
- Exceptional attention to detail organizational skills to manage multiple patients cases simultaneously.
- Proficiency in using electronic medical record systems and other relevant software applications, including CPR+.
- Ability to work both independently and as part of a team, demonstrating adaptability and strong problem-solving abilities.
- Experience in sterile compounding is considered an advantage.
Education and Experience Requirements
- A certified pharmacy technician license is strongly preferred.
- At least 1 year of experience in a Home Infusion pharmacy, specifically in the intake department working with TPN.

- Monday-Friday 8:00am-4:30pm
