Job Title: RISE Center Care Manager Starting Salary: $51,163
The case manager provides support to Oakland County residents at the Oakland County Resource
and Crisis Center (RCC). This position is a full-time office position which requires minimal travel. The
primary focus of the case manager position is to provide support, education, connection to
community resources, and advocacy to persons served. Some significant roles and responsibilities
include DHHS processes, various types of employment advocacy, and familiarity with the services
provided by Ooakland Community Health Network, including crisis services at the Resource Crisis Center.
Under general direction and as assigned:
- Be familiar and comfortable with the culture and characteristics of persons with substance use disorders and Mental Health disorders and provide a welcoming and accepting environment in which this population and their families can receive services
- Completes documentation and required paperwork within 24 hours of service delivery
- Have knowledge of the Michigan Department of Health and Human Services (MDHHS) programs and eligibility requirements
- Know the MDHHS MiBridges website and services available to clients through it
- Be familiar with community resources available to Oakland County residents
- Have knowledge of Oakland Community Health Network (OCHN) processes and help clients navigate mental health and substance use screening parameters
Education:
Bachelors of Social Work
Training Requirements (licenses, programs, or certificates):
· Bachelor’s Social Work License
· CADC or CAADC preferred
Experience Requirements:
- At least one year working in the human service field. School internships may be considered

- Have effective communication skills and be comfortable doing presentations
- Have knowledge of health insurance practices and be able to direct clients in using servicesavailable to them through their health plans
Knowledge Requirements:
- Knowledge of protocols related to acute care and transitions in care.
- Knowledge of services and supports in a crisis continuum of care.
- Knowledge of the Petition and Certification process.
- Knowledge of Recipient Rights.
