Position Responsibilities: The Health Care Integration Manager is responsible for advancing opportunities to align AgeSpan's social care services with the health care services of regional medical and behavioral health providers and payors. The Manager will have a significant focus on relationship and/or contract development with health care systems and organizations. The Manager is responsible for security new partner and payor relationships (private and nonprofit sectors) in market(s) designated by the AgeSpan executive leadership team. The Manager is involved in all aspects of the relationship cycle including identifying new health care prospects, qualifying prospects, making presentations, and, where appropriate, securing the contract. The Manager will work in close partnership with members AgeSpan's Executive Leadership team to ensure client satisfaction, to retain client relationships, and to advance new business opportunities.
Essential Functions: (Core duties or tasks that are fundamental and not marginal to the performance of the job)
Sustainable Health Care Alignment
- Must become knowledgeable to represent all AgeSpan social care products and services and work collaboratively with AgeSpan content experts.
- Implement AgeSpan social/health care strategies in our regional and statewide catchment area.
- Collaborate with Chief Strategy Officer and leadership team on research, identification, and preparation of solicitations for prospective AgeSpan payor relationships.
- Develop value propositions for social/health care alignment and translate these propositions into sustainable contracts and revenue streams.
Relationship Management
- Establish and maintain client relationships to ensure ongoing support, tracking all engagement in customer relationship management system.
- Collaborate with AgeSpan Executive Leadership and across all departments to ensure ongoing client communications, customer satisfaction, and opportunities for renewals and value-added business are addressed.
- Participate on cross-functional teams to give input on new products, services, marketing materials, and to curate opportunities for institutional fundraising supports based on client feedback and market needs.
- Lead and demonstrate AgeSpan's values, culture of business development and client engagement in all aspects of client activities.
Qualifications:
- Masters degree in nursing, non-profit management, public health preferred. Bachelors degree with equivalent experience considered.
- Significant knowledge of and experience in aging services policy and programs
- 3-5 years experience in state/federal aging services, complex health systems across the care continuum
- Customer service oriented, self-starter with ability to strategize, initiate, and deliver on new initiatives and maintain existing relationships.
- Strong written and oral communication skills, comfortable with public speaking and representing the organization to multiple stakeholders.
Hours : 37.5 per week
