Job description
River Edge is looking for an experienced license eligible or licensed Clinical Service Coordinator with a master’s degree in social work, Psychology, Counseling or Behavioral Health related fields for our Adult Outpatient Center to complete behavioral health assessments on primarily adult populations with mental health or dual diagnoses. Requirements
- Performs and tracks need for biopsychosocial and behavioral health assessments, treatment planning/monitoring and discharge planning, in person or via telehealth for clients participating in mental health group training and peer support groups.
- Completes assessments for appropriateness for group including referrals from NGRI/IST, parole/probation, region referrals, hospital discharges, and internal referrals.
- Assesses and refers to internal programs such as case management, substance abuse outpatient programs, supported employment services, early intervention program for individuals experiencing their first episode of psychosis, intensive case management, ACT services, residential programs, or crisis services.
- Collaborates with group leaders to develop treatment plans and develop appropriate interventions for individuals served in the group.
- Discharges and transitions clients to other programs accordingly
- Updates client records including contact information, ROIs, etc.
- Completes documentation or calls to report to personal care home, court, probation/parole, treatment teams, NGRI/IST and other partners.
- Completes report and attends monthly NGRI/IST meeting. (3rd Tues of each month)
- Attended monthly supported employment meeting and gives updates on individuals being served in group and supported employment.
- Performs 60 hours of productivity monthly.
- Refers client or family to community resources to assist in recovery from mental or physical illness and to provide access to services such as financial assistance, legal aid, housing, job placement/education, etc.
- Maintains safe, secure, and healthy work environment by following and enforcing set standards and procedures and complying with legal regulations.
- Adheres to professional standards, policies and procedures, federal, state, and local requirements, ASO, DBHDD, and Commission on Accreditation of Rehabilitation Facilities (CARF) standards.
- Updates job knowledge by maintaining licensure, participating in educational opportunities, reading professional publications, maintaining personal networks, participating in professional organizations, etc.
- Master's degree in behavioral health field AND eligible for Georgia license as LCSW/LMSW, LPC/APC, LMFT/LAMFT. Must actively pursue licensure upon hire.
