Job Description
The Supervisor of Felton Supportive Housing Program, located in Bayview, will lead a team of Case Managers and work closely with the Maintenance Supervisor. Oversee guidelines of HSH and HUD requirements. This position has variable weekend shifts to accommodate emergencies. This position will also help develop new headway providing structure, follow up, and possible development of Life skills and some supportive services. The position will include new implementations of new program policy and procedure, as well as working closely with the Director of Operations to provide training and quality assurance for staff working with Felton’s internal EHR System Circe and HSH One System. Coordinating, and integrating programs within the organization, the Supervisor will Facilitates team meetings and attend all other required County meetings with County stakeholders. Coordinate intakes, ensures a welcoming environment for all participants. Facilitate trainings for program staff. The supervisor will be responsible for meeting all program Contract requirements.
Program Specifics Responsibilities
- Responsible for program Implementation and continuous program improvement.
- Provides continues support to all case managers working with Felton’s Internal EHR System Circe and HSH One System.
- Ensure and provide tracking for all staff to receive required HSH training.
- Maintain and manage Quality Improvement control to strengthen program compliance and staff documentation.
- Implement and assist in the design of program delivery components, including annual recertification.
- Collaborate with HR Department with Post Positions, Hire / onboard new staff.
- Develop a culture of collaborative teamwork and accountability to increase staff engagement and commitment.
- Provide proactive and timely performance feedback and supervision of staff.
- Encourage open two-way dialogue with CM’s and participants of the housing program.
- Improving services to meet contract requirements.
- Strengthen existing partnerships with HSH.
- Oversee/reviews and ensures charting and compliance of all documentation standards for Case Managers completing Assessments and Treatment Plans of Care, in regard to HSH/HUD and Felton Standards.
- Overseeing, planning and recordkeeping, and day-to-day management of staff and office operations.
- Assist with trouble shoot and correct errors as needed, & Policies and Procedures are up to date.
- Conduct staff evaluations and monitor compliance with Felton and HSH contract.
- Coordinate outreach efforts in community., including providing presentations to All community.
- Attend All required County Meetings.
- Attends Felton’s Round Table meeting.
- Assist with running weekly staff meetings.
- Other duties as assigned.
Minimum Qualification
- Excellent written and oral communication skills.
- Commitment and ability to work as part of Felton’s leadership team.
- Demonstrated ability to utilize computers and proficiency in Microsoft Office software.
- Projects a professional demeanor and possess excellent writing and oral communication skills,
- A minimum of 2 years of experience
- High School diploma or equivalent to level of experience
- Previous related experience
- Ability to independently speak, understand, read, and write in English adequate to the meet the needs of the job.
- Must be able to lift-up to 60-100 pounds.
- Ability to follow instructions from management.
