Position Overview:
We are seeking a dedicated Case Manager to join our Guardianship Program team. This role involves providing compassionate and comprehensive case management services to our clients in the Merrimack Valley area of Massachusetts. As a Case Manager, you will play a crucial role in supporting disabled and elderly adults with their daily needs while ensuring they receive the highest quality of care.

Key Responsibilities:
Conduct regular face-to-face visits with assigned clients at their place of residence within Massachusetts.
Provide transportation for client visits and occasional travel to the Fall River office for supervision or staff meetings.
Manage all case management duties including hospital admissions and discharges, legal paperwork, property management, and monitoring of personal needs.
Maintain accurate documentation and billing records for each client.
Foster positive community relations with stakeholders such as social workers, physicians, and attorneys.
Assist in decision-making responsibilities for clients as needed.
Respond to phone calls, emails, and other communications in a timely manner.
Participate in On-Call Rotation to respond to emergency situations during weeknights and weekends.
Collaborate with fiscal department to ensure clients meet state income guidelines and manage health insurance and financial paperwork.
Complete MassHealth applications and redeterminations as required.

Qualifications:
Bachelor's degree in Psychology, Social Work, Sociology, or related field from an accredited university.
Prior experience working with disabled or elderly adults in a community or long-term care setting.
Experience with fiscal accounting or bookkeeping preferred.
Strong organizational skills with the ability to manage schedules and appointments effectively.
Compassionate and empathetic attitude towards clients' needs.
