Job description

The Registered Nurse functions as the patient care coordinator by developing adequate means and methods to facilitate implementation of patient care policies and procedures. Reports as scheduled. Demonstrates knowledge and competency in the performance of waive testing through training, updated knowledge of the manufacturers' manuals and annual observation of routine work by the supervisor or a delegate. Maintains confidentiality at all times.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties as required.

1. Works closely with the Director of Nursing and the Assistant Director of nursing to provide scheduling and coverage as needed throughout the facility. This will also include on-call responsibilities.

2. Initiates nursing process utilizing the nursing assessment tool on an ongoing basis, by planning coordinating, implementing and evaluating nursing care based on clinical knowledge, patient's needs and a collection of data.

3. Coordinates the nursing and medical needs as it relates to the patients receiving methadone and ensures all daily/dispensing reports are printed and filed so that they are available for review by state and federal agencies including JCAHO.

4. Provide weekly medication education groups on detox and ensure scheduling of and provision of groups on the rehabilitation unit, inclusive of documentation.

5. Develops and updates treatment plans to address nursing diagnosis and reviews with the multidisciplinary team to provide input for diagnosis, length of stay and readiness for the next level of care.

6. Implements detoxification procedure and delivers appropriate patient care according to withdrawal scale and physicians orders.

7. Retrieves orders and administers correct medications per physician's orders and facilitates procedure and policy. Maintains update knowledge of withdrawal medications and modalities used in addiction treatments.

8. Records and reports to appropriate care providers any information that may impact the patient's treatment and/or continuing care and completes medical discharge form.

9. Initiates consult(s) (psych, nutritional) when appropriate.

10. Assesses patient for readiness for transfer to next level of care.

11. Triage to minor injuries and illnesses of patients in coordination with the medical staff and RN Charge Nurse.

12. Assumes responsibility for growth and accountability through in-service programs and staff meetings.

13. Provides orientation and training for new employees.

14. All information maintained accurately and in compliance in electronic chart.

15. Participates in Performance Improvement Initiatives.

16. Complete Methadone verifications and documentation.

17. Responsible for reviewing intake charts for patient appropriateness for admission.

18. Morning and Mid-Day Med Passes.

19. Treatment planning with team

20. Groups (Med education as assigned)

21. Read and record PPD results

22. Triage Sick calls and provide dietary or psych consults as needed

23. On call for weekend coverage every four weeks.

24. Provide coverage if needed in the unscheduled absence of another Rehab nurse.

25. Other duties as assigned.
