Social Worker LCSW - Trinitas Medical Center
JOB SUMMARY & REQUIREMENTS:
- The LCSW will provide outpatient mental health and co-occurring treatment services to individuals who have a diagnosis of serious mental illness and a co-occurring substance use disorder,
- The LCSW will provide initial assessment, individual therapy, group therapy, referral and linkage,
- Design and implements wellness and recovery plans, including individual, group, and/or family psychotherapy, and evaluates and documents patient outcomes according to department policy and procedure,
- Interprets nature of illness and prescribed course of treatment to patients and families,
- Communicates family issues to other staff members,
- Participates in planning conferences,
- Maintain up-to-date and complete records of patient information according to established standards,
- As a member of the multidisciplinary team, assist in review
- Collaborates with other hospital programs and community agencies with which patients and families are involved,
- Facilitate linkages to outpatient services
REQUIREMENTS:
- Bilingual Spanish Required, LCSW Required,
- Experience with Co-occurring population a plus
