Position Summary:
We are happy to be implementing new programming through the HealthySteps Program. In conjunction with this service we are seeking a social worker to take on the capacity of HealthySteps Specialist to work within our pediatrics division and work in close conjunction with our clinical team in behavioral health.
HealthySteps is a pediatric primary care program that supports healthy early childhood development and. effective parenting. A child and family development professional, known as a HealthySteps specialist, connects. with families as part of the primary care team during pediatric well-child visits.
This is an onsite role based in our flagship health center in the Bronx, NY.
Minimum Qualifications:
- Master's Degree in Social Work coupled with 2 years of direct experience providing mental health/early childhood development-based services.
- NYS Licensed Masters Social Worker- LMSW is strongly preferred.
- Can consider Bachelor's level social worker incumbent who has advanced experience and expertise with early childhood development.
- Previous expertise working with families with children 0-3 years old.
- Ability to thrive in a fast-paced environment.
- Bilingual English/Spanish is strongly preferred.
Key Duties:
- Promotes HealthySteps within the practice to families and providers. With the team, implements and scales up developmental and other required screenings for patients ages 0-3 and their parents/caregivers to identify and provide services to at risk children.
- Administer and score screenings and make appropriate referrals to the Healthy Steps Specialist.
- Conducts team based well-child visits before, during or following the primary care provider focused on monitoring development, social-emotional functioning, and relational health and providing anticipatory guidance.
- Conducts consultations on a short-term basis for needs regarding development or behavioral concerns such as sleep, positive parenting, picky eating, etc. and consultation to providers on early childhood development, early relational/mental health, care coordination, goal setting and coaching.
- Maintains a child development support line and responds to and tracks call requests within the designated response time.
- Participates in reflective supervision meetings. Documents all patient contacts as required and escalate crisis cases to the supervisor.
- May conduct home visits and may accompany to key medical, specialty, and community agency appointments as needed and facilitate parent/caregiver groups as needed.
- Creates and maintains a community resource directory/database. Provides referrals and tracks follow up to facilitate linkage to needed services working with community providers attending meetings as needed.
- Tracks caseload ensuring risk stratified model is implemented and that patients are connected to needed services internally and externally including higher level of care if needed.
- Analyzes data on screening and referral rates and work with the pediatric and quality team to increase screenings and improve workflows and policies.
- Participates in required trainings and meetings with both the Healthy Steps National Office and with Urban Health Plan's pediatric and quality team ensuring fidelity to the Healthy Steps Program model and continuous quality improvement.
- Others as required by the organization.

Starting Compensation (Based on education, licensure, and experience):
With Master's Degree/LMSW: Minimum $80,000 per year; Maximum $85,000 per year.
