The RN Case Manager provides service to individuals (in their own homes and communities) who need assistance caring for themselves as a result of old age, sickness, disability and/or other afflictions.

Current nursing (RN) license in State of Illinois
Minimum 1 year of clinical nursing experience
Minimum 1 year case management experience
Current Driver’s license, current auto insurance, working car
Clean background and criminal record

Your Job Responsibilities:
Conduct Initial Nursing Assessment within 48 hours of start of care
Conduct "meet and greet" introductions on new field staff placements
Monitor and coach home nursing and home services field staff
Review weekly care notes for quality of documentation, changes in patient condition requiring revisions to Plan of Care
Case management: contact families, coordinate care, complete field visits in homes and facilities, communicate changes in Plan of Care. Obtain physician’s signature approval if necessary.
Foster trusted relationships with peers and referral sources in facilities
Administer prescribed medications and treatments in accordance with approved nursing techniques
Conduct field supervisory visits, perform competency reviews on field staff skills as needed
Perform field injections, medication set ups, blood draws, infusions
Observe patient, record significant conditions and reactions, and notify supervisor or physician of patient’s condition and reaction to drugs, treatments and significant incidents
Take and record patients’ vital signs to detect deviations from normal and assess condition of patient
Respond to life saving situations based on nursing standards, policies, procedures and protocol
Document nursing history and physical assessment for assigned patients
Initiate a patient education plan according to the individualized needs of the patient as prescribed by the physician and/or agency policy including patient and family instruction
Participate in the implementation and evaluation of patient care
Participate in weekend on call rotation
Available to clients and families 24/7 for their healthcare needs
