Job description

Our Safe Haven Integrated Health Home team provides supportive services to individuals challenged by homelessness, substance use and mental health issues. We use a harm reduction, person-centered approach. You will work with a team of professionals - a c linician, prescriber, nurse, and case manager, all of whom work in concert to provide the highest quality care. You will p rovide clinical and support services for individuals and their families who are coping with long-term needs associated with homelessness, mental illness and substance use.  Develop, design and implement person-centered recovery plans  to help people maintain their health and well-being. We need someone who can d emonstrate strong writing, communication, computer skills, and documentation skills. You must have  the ability to multitask, be organized, and have excellent time management skills. T ransportation of clients in your vehicle to medical and community appointments is required.
