Growth Outreach Specialist – ACO REACH
Position Description
Overview The Growth Outreach Specialist for ACO REACH reports to the operational leader in their assigned market with accountability for supporting growth related activities for the ACO REACH model. The Growth Outreach Specialist is accountable for supporting their assigned market(s) in the management and execution of CINQCARE’s growth-oriented programs such as voluntary alignment in their assigned market(s). The Growth Outreach Specialist will be responsible for: collaborating with the markets team and participating practices to support patient engagement activities, namely voluntary alignment, including both direct and indirect engagement of eligible beneficiaries in their community. The Market Outreach Specialist should enjoy engaging with medical practices and Medicare beneficiaries to better understand the benefits available to them through their insurance coverage, the ACO REACH program, and CINQCARE. we have Family Members. Job Responsibilities The Growth Outreach Specialist will have the following responsibilities:
- Work closely with the practices in your assigned market(s) to engage eligible beneficiaries and encourage them to come in for healthcare services.
- Support in the collection and management of mission critical documents with regard to our programs, including voluntary alignment for Medicare beneficiaries.
- Monitor the success of outreach campaigns in your assigned market(s).
- Work with national ACO REACH team to refine best practices and approach to voluntary alignment/new patient engagement in your market(s).
- Assist in the research and development of best practices and strategies regarding voluntary alignment and other growth-oriented initiatives as assigned.
- Ensuring secure maintenance and organization of documents on behalf of CINQCARE ACO REACH for your assigned market(s).
- Provide regular updates, respond to inquiries, and address all internal and external concerns in a timely manner.
- Provide administrative, operational and project support, as needed.
- Assist with all other required business needs as assigned by the manager.
General Duties The Growth Outreach Specialist will have the following duties:
- Leadership : The Growth Outreach Specialist will lead in defining and executing best practice workflows for practices to identify and engage eligible beneficiaries .
- Strategy : The Growth Outreach Specialist will establish the business strategy and roadmap to ensure that practices have defined a successful and effective voluntary alignment process.
- Collaboration : The Growth Outreach Specialist will ensure that ACO REACH capabilities form a cohesive offering, including by working closely with other business divisions to learn their needs, internalize their knowledge, and define solutions to achieve the business objectives of CINQCARE.
- Knowledge : The Growth Outreach Specialist will provide subject matter expertise in ACO REACH solutions, including determining and recommended approaches for ACO REACH, solution deployment and performance evaluation .
- Culture : The Growth Outreach Specialist is accountable for creating a productive, collaborative, safe and inclusive work environment for the ACO REACH team and as part of the larger Company.
Qualifications The Growth Outreach Specialist should have the following qualifications:
- Education: Bachelor’s or equivalent degree with a record of strong academic achievement. Prior experience will be considered in lieu of degree ;
- Experience: Ideal candidates will have 2+ years of relevant experience in healthcare although previous healthcare experience is not a requirement; Previous experience working to Identify community health needs, conducting outreach programs, and connecting individuals to relevant health services a plus. Candidates who are passionate about acting as advocates for individual and community health needs within their community and have experience building relationships with other health professionals and organizations to facilitate access to services strongly preferred ;
- Entrepreneurial: CINQCARE seeks to fix gaps that have persisted for generations in the delivery of care to Black and Brown populations. This position is accountable for ensuring CINQCARE is positioned to innovatively deliver on its promise.
- Communication: Excellent verbal, written communication and presentation skills; ability to clearly articulate and present concepts and models in an accessible manner to CINQCARE’s team, investors, partners, and other stakeholders. Proficiency in all Microsoft Office applications.

Location : New Jersey
Compensation : $58,458.40 - 65,000 annually
