Job description
Corrielus Cardiology seeks a Nurse Practitioner to join its expanding practice. Dr. Corrielus is seeking a trusted clinician to join him at SNFs and Assisted Living settings to ensure the patients needing care receive the best patient care model his practice renders at his main practice location. This role will not offer health benefits.
Duties/Responsibilities: - Prescribes medication, examine patients, diagnose illnesses, and provide treatment.
- Orders, interprets and evaluates diagnostic tests.
- Gathers health histories and performs physical examinations and diagnostic procedures.
- Develops and implements treatment plans.
- Notify the CEO about unusual or unexpected events or problems.
- Effectively communicates with diverse populations, including physicians, employees, patients, and families.
- Provides for continuity of effective patient and family care by exchanging information and cooperating with workers of other community health and welfare agencies concerned such as hospitals, schools, and nursing homes.
- Assists families in achieving healthful, safe living for all age groups by giving instructions and guidance in the basic principles relating to accident prevention, communicable disease control, nutrition, child growth and development, and physical and emotional fitness.
- Aids individuals and families to accept and adjust positively to physical, mental, and social limitations.
- Provides direct clinical care for the patient population according to established protocols and utilizes evidence-based practice standards in collaboration with the clinical team.
- Maintains accurate and confidential medical records. Documents all medical evaluations, diagnoses, procedures, treatments, outcomes, education, referrals, and consultations consistent with organizational standards in the electronic health record (EHR).
- Participates in the performance improvement processes to meet quality of care and service standards.
- Demonstrates awareness of professional responsibility as a role model to peers and practice staff team members.
- Performs approved therapeutic or diagnostic procedures based on the patient's clinical status.
- Provide patient education, counseling, and instruction regarding patient diagnosis, medical treatments, medications, and preventative care.
- Participates in and supports the compliance and regulatory activities of the practice.
- Identifies patient/family needs for community resources and refers appropriately.
Required Skills/Abilities: - Population Health/Community Outreach experience
- Knowledge of professional nursing principles, techniques, basic sciences, and of community health and welfare resources.
- Ability to apply professional nursing principles, techniques, and basic sciences in homes, clinics, and the community.
- Ability to carry out written and oral instructions and exercise judgment in applying nursing care.
- Ability to work cooperatively with people and to maintain an objective attitude.
- Ability to exercise tact and initiative.
- Excellent written and verbal communication skills.
Credentials: - Advanced Cardiac Life Support (Required)
- BLS/CPR, as a healthcare provider as per the American Heart Association (For providers in the ambulatory setting) required.
Education or Equivalent Experience: - Master of Arts or Science (Required)
- Family Nurse Practitioner certification (preferred)
- Master's Degree or Doctor of Nursing Practice (DNP) in Nursing preferred.
