Social Worker - PRN
What You Must Have: - Bachelors or Master’s Degree in Social Work from a State accredited program
- Current State license required in accordance with State regulations
- Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance
- One year of experience as a Social Worker in a clinical care setting required, hospice experience preferred

What You Will Do: - Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs as evidenced by POC documentation, clinical records, team meeting minutes and community resource referrals.
- Provide individual patient/family counseling, crisis intervention, information and assistance with preparation of advance directives, funeral planning issues and transfer of responsibilities relative to legal and fiscal issues.
- Facilitate community resources based on patient/family needs.
