Position Overview:
We are looking for a Palliative Nurse Practitioner who will play a crucial role in providing specialized care for patients facing serious illnesses. As a member of our home health team, you will work closely with patients and their families to create individualized care plans that prioritize comfort, symptom management, and overall quality of life.
Job Duties:
- Conduct thorough assessments of patients' medical conditions, pain levels, and psychosocial needs.
- Collaborate with interdisciplinary teams to develop and implement palliative care plans.
- Provide expert guidance on pain and symptom management, as well as emotional support for patients and their families.
- Monitor and evaluate the effectiveness of palliative care interventions and adjust plans as needed.
- Educate patients and families on treatment options, prognosis, and available resources.
- Coordinate with other healthcare professionals, including physicians, nurses, and social workers, to ensure comprehensive care.
Qualifications and Experience:
- Valid (unencumbered), current Nurse Practitioner license in Utah.
- Certification in Palliative Care preferred.
- Minimum of one year experience as a NP, with a focus on palliative care.
- Current CPR Certification.
- Valid driver's license, reliable transportation, and agency-required liability insurance to complete home visits.
- Proven experience in case management of care coordination.
- Strong interpersonal, communication, and organizational skills.
- Empathy, compassion, and a patient-centered approach to care.
- Proficient in using electronic health records (EHR) and other healthcare software.
- DACS clearance required (background, fingerprinting).
Physical Requirements:
- Mobility: The Palliative Care NP should be able to move around freely and comfortably, as the job involves traveling to different patient locations, such as homes, and independent/assisted living facilities.
- Lifting: The Palliative Care NP might need to lift and maneuver equipment, supplies, and occasionally assist patients. Ability to lift in excess of 50 lbs. Adequate strength and mobility are important to prevent injury.
- Walking: Walking and standing are common when visiting patients' homes, and independent/assisted living facilities.
- Comfortable footwear (closed toe) is essential for extended periods of standing and walking.
- Bending and Reaching: The ability to bend, stoop, and reach is important for tasks like assessing patients, assisting with treatments, and organizing supplies.
- Manual Dexterity: Good hand-eye coordination and manual dexterity are necessary for tasks such as administering medications, dressing changes, and handling medical equipment.
- Visual Acuity: Clear vision is essential for accurately reading patient records, assessing medical charts, and ensuring proper medication administration.
- Hearing: Adequate hearing is important for effective communication with patients, families, and colleagues, especially in a variety of environments.
- Endurance: The job can be physically demanding due to the combination of travel, patient care, and administrative tasks.
