A little about us...Partners In Home Care, a highly-respected home health agency based in Missoula, Montana, is seeking to hire a few folks who want to reinvigorate their nursing career working with a highly professional healthcare team. We offer more than a job; we will help you build and sustain your career. Partners In Home Care is the premier home health agency in Montana. With a staff complement of 200 strong, we serve a radius of 100 miles around Missoula with a broad array of professional healthcare including nursing, occupational therapy, physical therapy, social work and bereavement support. Our programs encompass Home Health, Hospice (the largest Hospice in the community), Home Infusion, Home Medical Equipment (including home modifications for those who want to age-in-place) and HCBS (the Medicaid "waiver" program that supports those who wish to live with independence).

We support you... with an excellent Education Team who ensure your learning is on track, with in-the-moment clinical support to answer your questions about your patients' needs, with an Annual Skills Fair to stay sharp, and with a patient triage service for on-call support which handles most patient needs during the on-call shift.

This is a full-time position with a business hour visit schedule and a rotating schedule for on-call and holiday coverage.

Our RNs put the patient at the center of care... They work with a inter-disciplinary team to provide skilled nursing care, and case management for patients in their homes. They coordinate services to ensure continuity of care for patients and their families. They manage patient care through a staff of professional healthcare providers including Home Health Aides, Social Workers, Chaplains, and other team members. Through patient and family instruction and evaluation, they provide the highest quality care and attention. They maintain accurate, current and complete patient records to assist with patient outcomes. On-call scheduling is assisted through a first-contact service that we contract with to minimize the disruption in our nurses lives; the service reduces the number of calls by approximately 50%.
