Job description
At 315 Health, we are dedicated to providing comprehensive healthcare services to our patients. We are currently seeking a highly skilled Registered Nurse (RN) to join our team as an Annual Wellness Visit (AWV) Nurse. As an AWV Nurse, you will play a vital role in helping our patients achieve and maintain their optimal health through preventive healthcare services. Responsibilities:
Conduct virtual Health Risk Assessments before Annual Wellness Visits for Medicare patients
Evaluate patients' health status and medical history
Perform various assessments and screenings, including cognitive evaluations, depression screenings, and falls risk assessments
Document and identify patient needs and identify areas for patient growth and improvement
Manage and update patients' health records and documentation
Make referrals to community support and resources
Evaluate for and educate on Continuous Care Program eligibility
Collaborate with healthcare teams to ensure coordinated and comprehensive care
Stay up-to-date with the latest guidelines and regulations regarding Annual Wellness Visits
Requirements
Active and unrestricted Georgia RN license (compact preferred)
Minimum of 2 years' experience as a Registered Nurse
Strong clinical assessment and critical thinking skills
Excellent communication, organizational, and interpersonal skills
Ability to work independently and as part of a healthcare team
Proficient computer skills and experience with electronic health records
Attention to detail and accuracy in documentation
Availability Monday - Friday during normal business hours
