DAY TO DAY AS A SERVICES COORDINATOR/CASE MANAGER

In this dynamic and rewarding role as a Service Coordinator at Community Living Case Management, you will have the opportunity to work closely with individuals and their family/support team who are eligible for case management services with Intellectual and Developmental Disabilities (I/DD). Your main responsibility will be to provide comprehensive case management services for I/DD individuals and their families, assisting them in accessing necessary services and resources. By developing, coordinating, and monitoring Individual Support Plans (ISPs), you will play a crucial role in ensuring that the unique needs of each individual are met. Collaborating with partnering agencies, local education associations, law enforcement, and other community organizations, you will serve as an advocate and liaison, helping clients navigate the complex landscape of services and resources.
