Job description

Position Description:

Referrals:

Assists in the coordination of home health services by working with the clinical management or coordinators for all departments, clinical staff, provider offices, hospitals, and all other referral sources. Updates liaisons with hospitalizations of patients as needed.
If clinical licensure allows; receives and processes physician"s verbal and written orders; documents per process and communicates as necessary to clinical staff or managers.
If clinical licensure allows; receives and processes SBARs; documents per process and communicates as necessary to clinical staff or managers.
Receives and coordinates referrals for all programs including: home care, perinatal, IV, rehabilitation, social service, support services, hospice, community palliative care, and telehealth.

Medical Records:

Maintain and update electronic medical records
Respond to medical record requests
Update medical records by reviewing information, notifying providers of record deficiencies, and tracking outstanding records.
Handles requests from staff to fax information to care partners.
Cover other medical records staff time off

General:

If clinical licensure allows; Triages calls; patient problems, physician orders, SBARs, scheduling changes and field staff concerns.
Able to efficiently use different forms of technology at a fast pace with minimal errors or corrections.
Responsible for participating in CVNA"s emergency preparedness processes and emergency building alerts.
Fulfills additional responsibilities as assigned and as necessary, accurately and in a timely fashion.
Participates in personal growth and development.
Requirements:

Two or more years of homecare experience preferred
Saturdays are required
Other Requirements:

Strong computer skills: internet portals, email, using multiple screens and portals at one time
Flexibility to cover weekends/holidays/during vacations
Strong interpersonal and communication skills
Critical thinking, problem solving and great organizational skills a must
Independence and initiative
Exceptional assessment skills
Computer competency, proficiency with medical EMR
Clean and valid driver"s license along with vehicle insurance are required
