Job description
We are seeking LTC MDS Coordinators to support a nonprofit LTC facility in Geneva NY.
This is an onsite role and relocation assistance is approved on a casebycase basis by the company.
Job Summary:
Under the direction of the Director of NursingLTC coordinates all Medicare assessments care planning and reimbursement activities for the facility to assure compliance with regulatory agencies.
Key Responsibilities:
- Assures completion of MDS in accordance with regulatory requirements including: interpreting new standards scheduling and communicating schedule to IDC team.
- Attending care plan meetings as needed.
- Assuring completion of CAAs and submission of MDS data per regulation.
- Monitoring and interpreting Quality Measures.
- Effectively coordinates reimbursement activities for the facility including: makes reimbursement decisions completes denial protocol and maintains records.
- Acts as a liaison between Nursing Therapy and Resident Accounts.
- Provides documentation oversight and assuring compliance including: reimbursement guidelines requirements identifying documentation needs and reviews with the Nurse Manager/DON and provides in servicing as appropriate.
- Submission of MDS census data collection for Medicaid reimbursement.
Requirements
Education:
- Associate degree in Nursing (required)
- BSN (preferred)
Required:
- NY RN license (current)
- 25 years of relevant MDS and care planning experience.
- Familiarity with documentation requirements for reimbursement.
- Knowledge of New York State Department of Health regulationsand survey process.
- Demonstrated leadership abilities
- Demonstrated ability to handle confidential information with discretion and ability to deal with the public in a professional and courteous manner.
- Ability to meet deadlines manage multiple priorities and enhance the spirit of teamwork through effective role modeling.

- Experience with Microsoft Office products and MDS/care planning software

The anticipated base pay range for this position is $70000 to $80000. This range may be modified in the future and actual compensation may vary from posting based on geographic location work experience education and/or skill level. Employees may be eligible to participate in company employee benefit programs such as health insurance savings plan pension plan disability plan vacation pay sick time holiday pay and work personal and family time off in accordance with the terms of the applicable plans.
