INS AND OUTS OF THIS ROLE

• Master's degree in social work, psychology, or a related field
• 3+ years of experience in a counseling or mental health position
• 3+ years of case management experience
• COVID-19 vaccination

Bilingual fluency in English and Spanish would be preferred.

As an Intensive Care Coordinator, you provide invaluable support and guidance to individuals in our IMPACT program. Your responsibilities include conducting screenings and intakes, orienting clients about program goals and regulations, and collaborating with a multidisciplinary team. You are responsible for thorough assessments, developing individualized service plans, and determining the appropriateness of existing care provided by mental health providers.

Your duties also involve conducting individual and group counseling sessions, crisis intervention, and managing client caseloads. Every week, you visit Rikers Island to screen clients due to be released and help plan their transitions. Accurate record-keeping and compliance with regulatory standards are essential to this role.
