Admissions Liaison Assistant
Day Shift
**Pay Rate up to $22/hour; based on experience**

Responsibilities of an Admissions Liaison Assistant:
- The ALA will review BGRV resident census information to keep informed of room/facility availability.
- They will occasionally attend daily stand-up meeting as needed at BGRNC to develop and maintain good working relationships with the other teams and to communicate pertinent information.
- They will establish strong, effective external relationships with area hospitals and other community-based entities to promote referral development and admissions.
- They will make rounds at local hospitals as needed to assist liaisons with potential admission referrals and to help cover for liaisons when away.
- As needed, follows hospitalized BGRV residents to determine discharge plan and needs, and ensures the provision of timely clinical information to appropriate BGRV facility prior to readmission.
- Responsible for case management rate determination with outside payers depending on the potential admission's medical needs; documents on appropriate forms.
- May meet potential residents and/or family members in hospital or at BGRV to establish rapport, educate on Medicare services, provides appropriate pre-admission paperwork, and discuss facility services, features and long-term room rates (when applicable).
- Coordinate with Liaisons and Admission Coordinators to arrange for tours of skilled nursing or Assisted Living.
- Conducts and participates in company open-house events as scheduled. Assists Marketing Department with trade shows and seminars as needed.
- Attends and participates in staff meetings, seminars, and trainings as scheduled.
- Participates in admissions team on-call rotation as scheduled.
- Interacts with residents, visitors, staff, and general public in a professional and helpful manner while performing daily duties.
- They will maintain inquiry, admission and referral source information on appropriate forms and EHR to facilitate communication in the department and to other departments.
- Assist with follow up to physicians for Assisted Living assessments to help ensure timely completion of the pre-admission state required assessment.
- Has the ability to work assigned work hours
Requirements of an Admissions Liaison Assistant:
- Licensed Practical Nursing degree; or one to two years related experience and/or training; or equivalent combination of education and experience; proficient in reading and understanding medical terminology and clinical records.
- Must possess a valid driver's license.
