Department: St. Joseph County Department of Health – Nursing Unit

Incumbent serves as the Immunization Outreach Coordinator for the Nursing Unit of the St. Joseph County Department of Health, responsible for coordinating and implementing immunization public health services and programs, collaborating with other Units and community partners to address public health issues, and providing direct health services.

 

DUTIES :

Provides direct services concerning immunization access through discussions with clients, scheduling appointments, ensuring access to interpretation, and addressing barriers to access.

Organizes mobile immunization clinics and outreach events. Serves as primary point of contact for community partners and develops and implements process for responding to organizational partner requests for mobile clinics.

Collaborates with other Department units to assess health status and social determinants of health, facilitate health education and behavior change strategies, and provide social service resources at outreach events.

Provides advocacy to address health disparities at individual and community levels. Utilizes creative ideas to reach out and contact underserved community members throughout the County.

Collaborates with existing community partners and establishes and nurtures new partnerships.

Assists with effectively supervising and directing the work of assigned staff, including interviewing candidates, orienting new employees, planning, and delegating work assignments, and keeping supervisor and assigned staff informed of organization developments.

Manages assigned personnel and logistics for day-to-day operations of assigned projects or clinics. Prepares staff work schedule, making changes as necessary. Maintains up to date information on clinic topics and disseminates to staff. Maintains related records and files, manages issues with medical records systems, and submits required documents to Indiana Department of Health (IDOH).

Maintains inventory of ancillary supplies for immunization clinics, submits request to Administrator for supplies, when necessary, receives and properly stores supplies, and coordinates distribution of supplies with Mishawaka location.

May provide technology assistance to co-workers and implements updates to new technologies. Contacts IT Department when necessary.

Assists with securing grant funding, including providing required documentation, reports, and other information in a timely manner, as required.

Provides other assistance with daily operations of immunization clinics, as directed.

Attends and participates in departmental and unit meetings.

Performs related duties as assigned.

JOB REQUIREMENTS :

Bachelor’s degree required with at least one year of coordination experience, and with at least one year of community-based advocacy and support with significant public contact, or equivalent combination of education, training, or experience considered.

Must be at least 21 years of age.

Possession of or ability to obtain and maintain required certifications and training.

Ability to meet all hiring and retention requirements, including passage of drug test.

Practical knowledge of public health guidelines, regulations, and accepted principles and practices, with ability to effectively research related resources and provide information, assistance, and referrals as appropriate.

Practical knowledge of standard policies and practices of St. Joseph County Department of Health, with ability to apply appropriate procedures accordingly and put into effect changes in policies and procedures.

Working knowledge of local community health disparities and local population demographics, assets, needs, and health and social services available to County residents, with ability to implement public health programs and facilitate referrals as appropriate.

Working knowledge of standard office policies and procedures with computer skills, including word processing, spreadsheet, presentation, email, internet, and Department-specific software systems such as medical records management systems, and ability to apply such knowledge to a variety of interrelated processes, tasks, and operations.

Working knowledge of filing systems with ability to create and maintain accurate and complete Department files.

Working knowledge of standard English grammar, spelling, and punctuation, and ability to prepare documents, correspondence, and detailed reports as required.

Working knowledge of grant administration and ability to assist with meeting all grant requirements on schedule.

Ability to assist with effectively supervising and directing the work of assigned staff, including interviewing candidates, orienting new employees, planning, and delegating work assignments, and keeping supervisor and assigned staff informed of organization developments.

Ability to effectively listen, comprehend, and communicate with co-workers, other County and City departments, state departments and agencies, community and social service organizations, community partners, schools, non-profits, and the public, including being sensitive to professional ethics, gender, cultural diversities, and disabilities.

Ability to properly operate standard office equipment, including computer, telephone, copier, and fax machine.

Ability to understand, memorize, retain, and carry out written or oral instructions and present findings in oral or written form.

Ability to compare or observe similarities and differences between data, people, or things, and apply knowledge of people and locations.

Ability to count, compute, and perform arithmetic operations.

Ability to compile, analyze, and evaluate data, make determinations, take action based on data analysis, and manipulate or fabricate data to discover facts or develop concepts or interpretations.

Ability to plan and layout assigned projects, work on several tasks at the same time, often amidst distractions and interruptions, and exhibit flexibility in prioritizing tasks.

 Ability to plan and deliver public speaking presentations and special events.

Ability to provide public access to or maintain confidentiality of Department information and records according to state requirements and Health Insurance Portability and Accountability Act (HIPAA).

Shall comply with all employer and Department policies and work rules, including, but not limited to, attendance, safety, drug-free workplace, and personal conduct.

Ability to occasionally work extended, evening, and/or weekend hours and travel out of town for meetings and conferences, sometimes overnight.

Possession of valid driver’s license, demonstrated safe driving record, and ability to operate mobile clinic vehicle.

II. DIFFICULTY OF WORK :

Incumbent performs a broad range of duties involving complex and complicated considerations and problems. Incumbent must frequently create new guidelines and approaches to deal with unique and novel circumstances that occur.

 

III. RESPONSIBILITY :

Incumbent’s performance may have a major impact on overall department operations and functions, and work is periodically reviewed for achievement of desired results, which are clearly specified, and conformity with overall policies and procedures of the department. Unusual problems or situations are discussed with the supervisor.

IV. PERSONAL WORK RELATIONSHIPS :

Incumbent maintains frequent communication with co-workers, other County and City departments, state departments and agencies, community and social service organizations, community partners, schools, non-profits, and the public for the purposes of exchanging information and rendering service.

Incumbent reports directly to Director of Nursing.

V. PHYSICAL EFFORT AND WORK ENVIRONMENT :

Incumbent performs duties primarily in a standard office and medical clinic environments, involving sitting/walking at will, sitting for long periods, pushing/pulling objects, bending, reaching, crouching/kneeling, keyboarding, lifting/carrying up to 50 pounds, handling/grasping/fingering objects, close/far vision, color/depth perception, hearing sounds/communication, speaking clearly, and driving. Incumbent is exposed to irate or difficult persons and normal health hazards associated with public health nursing for which universal health precautions must be followed to ensure safety of self and others, including wearing protective clothing or equipment. 

Incumbent is required to occasionally work extended, evening, and/or weekend hours and travel out of town for meetings or conferences, sometimes overnight.
