THE GROWTH YOU HELP PROVIDE

As an LPN Nurse - Licensed Practical Nurse within our Care Management team, your typical day is a dynamic blend of patient care and essential coordination. Your primary focus is on supporting the Medical Provider (MD and NP) by following their schedule and preparing clients for appointments. You'll engage in various integrated care functions, acting as a liaison with external providers, facilitating medical scheduling and reminder calls, coordinating comprehensive care, and ensuring referrals to social support systems. Beyond this, you'll play a pivotal role in patient and family support, transitional care, health promotion, and the completion of critical quality measures.

In addition to these responsibilities, your role extends to critical medical functions, including performing routine medical tests, administering prescribed treatments and medications, recording vital signs, and conducting routine laboratory tests. Your meticulous documentation and commitment to patient care in a culturally sensitive and non-discriminatory manner are vital. You'll also be involved in tasks such as managing medication-related phone calls, obtaining prior approvals, addressing abnormal lab results, and overseeing inventory when necessary. Your dedication to upholding the highest standards of patient care while fulfilling these multifaceted responsibilities is central to our mission of delivering exceptional healthcare services.

QUALIFICATIONS

Experience working with psychiatric clients is preferred but not required.
