Health Services Care Manager

Are you looking for a meaningful job that can truly make a difference in someone's life, then come join the Mystic Valley Team! The Health Services Care Manager/Case Manager is a community-based position where you can build relationships with older adults by providing case management.

Schedule: This is a Local Union 509, full-time, non-exempt (35 hour/week) position. We offer 4-day work week and hybrid work schedule (once training is complete.) No nights or weekends.

Compensation: Starting annual salary is $47,274.86. Being bilingual is not a requirement for this role, but we recognize it as an asset and offer an additional 6% bilingual designation for Spanish, Haitian Creole, Russian, Vietnamese, Cantonese or Mandarin.
