Job Description:

You will play a crucial role in supporting children and families through a range of family support and advocacy services. Your responsibilities will include conducting child development screenings and assessments, developing individualized school readiness plans for children, and conducting weekly home visits. Additionally, you will assist in facilitating socialization groups, participating in team meetings, and providing support to other team members. Your work will be instrumental in ensuring that families receive the resources and support they need to thrive.

Key Responsibilities:

Conduct comprehensive child development screenings and assessments
Develop individualized school readiness plans for children
Conduct weekly home visits and assist with socialization groups
Develop Family Partnership Goals and assist families with community resources
Participate in team meetings to implement goal-oriented family plans
Maintain documentation of services to meet program standards
Provide support and mentoring to team members
Essential Functions:

Home Visiting:

Develop a regular schedule of home visits and activities
Interact positively with children and families to support learning and development
Utilize Parents as Teachers curriculum to facilitate caregiver-child interaction
Developmental Screening and Assessment:

Conduct screenings and assessments with family input
Maintain ongoing documentation of observations and assessments
Individualized School Readiness Plans (ISRP):

Develop ISRP for each child based on assessments and family input
Identify family strengths, needs, and interests
Family Support and Parent Involvement:

Provide appropriate referrals and support through community resources
Participate in planning and attending family events
Community Resource and Referral:

Maintain awareness of community resources and build collaborative partnerships
Transition Services:

Support successful transitions for children and families
Record Keeping:

Maintain complete and confidential records for children and families
Staff Training:

Attend required trainings and provide on-the-job support for new staff members
Other:

Advocate for the values of diversity within the organization
Perform other duties as assigned by Supervisor
Knowledge, Skills, and Abilities:

Ability to establish positive relationships with individuals and families
Competence in planning, organizing, and communication
Knowledge and experience in child development and family dynamics
Skilled in motivating families and understanding family support principles
Proficient in MS Office and data entry
Work Environment:

Split between clients' homes, office, and car
Ability to work remotely based on position requirements
Physical Demands:

Ability to sit on the floor, lift children if necessary, and sit in a car frequently
Minimum Qualifications:

One year experience in home visiting or Early Learning Program
Access to a personal vehicle with insurance coverage and valid driver's license
CPR and Pediatric First Aid certification
Completion of criminal history background check
Preferred Qualifications:

Home Visitor CDA credential or Associate degree in Early Childhood Education
Bilingual in English and another language is highly preferred
Salary Range and Benefits:

Salary range: $22.54 - $30.67
