Women's Health Coordinator
Southern Jersey Family Medical Centers, Inc. is looking for a
talented Women’s Health Coordinator to step into our culturally
diverse and fast-paced Hammonton, NJ facility!
Summary: The basic function of the Women’s Health
Coordinator is to provide patients with continuous, coordinated and
comprehensive health services to meet that individual’s needs by
coordinating Health Education with combined services from the nursing
staff, Nutritionist, Obstetricians and the Social Worker. This
position comes with a Sign-on bonus of $1,500 after 6 months of
satisfactory performance!
Hours: This is a full-time position that requires the
ability to work a flexible schedule. General daytime hours Monday -
Friday with 1 to 2 evenings per week.
Position Responsibilities/Duties:
- Orients obstetrical patients upon the first registration visit
regarding the content and process of comprehensive maternity care services
- Informs the patient of what services will be provided, who will
provide these services, where to obtain these services, when to go
for each given service, and who to contact in the event of
complications arising from these services
- Arranges all patient services and linkages with appropriate agencies
- Develops an individual care plan for each patient
- Implements case conferences when necessary, with other care
providers, to implement the care plan
- Performs all record keeping duties related to maternity care
services and chronic case management services
- Provides, coordinates, reinforces and supports all health
teachings with patients as needed
- Obtains all information regarding labor, delivery and post-partum
records within two weeks of delivery and coordinates all post-partum visits
- Provides coverage to all OB patients as needed
- Perform other tasks as assigned by the Director of Nursing,
Obstetrics Program Administrator and/or the President/CEO
Experience and Skills:
- At least 2 years of related work experience in a medical setting
- Working knowledge of electronic medical records and referrals
- Knowledge of medical terminology and office procedures
- Excellent verbal and written communication skills
- Bachelor’s Degree required, Healthcare/Social services,
Psychology, Social Work, CHES or Public Health
