Job Duties:
A Nurse Specialist is needed to work in the Pediatric Outpatient clinical area. The nurse will serve in a team based capacity providing care coordination for a designated patient pool. The Care Coordination role to include special focus on identified panel of complex children. The nurse manages comprehensive nursing and case management for parents and caregivers. This position works as part of an interdisciplinary team of providers, nurses, social workers, referral coordinators, therapy companies, and home health agencies to identify potential needs and solutions for optimal patient care. The nurse must maintain clear and effective ongoing communication with providers. Methods of communication include documentation in the electronic health record, in basket messaging and oral communication to ensure continuity of care for patients. This position will speak with parents by telephone and face-to-face to address concerns, questions, obstacles, and care needs. The nurse is responsible for scheduling, organizing, and managing all aspects of the patient's healthcare maintenance and treatment. The selected candidate will work closely with the Office of Quality and Analytics to assist with quality projects for ECU Health Physicians-Academic Practice such as Meaningful Use, Healthcare Effectiveness Data and Information Set, Merit-Based Incentive Programs, and Physicians Quality Reporting System by closing care gaps. This will be accomplished by the review of records, patient education, and ensuring that referrals for preventive health services are scheduled and documented to close existing and potential care gaps. This position will be responsible for reviewing reports and data to identify patients with care gaps and personally reaching out to schedule patients with the provider, health coach, and/or other care management resources. Identifies and improves throughput issues that impact patient access. Organize and prioritize nursing care activities considering the needs of the patients and other staff members to facilitate an efficient flow from patient check in until the end of the doctor's office visit with a team-based approach to care. This nurse will also serve in the preceptor role for the clinical area as the continuity team nurse. Uses formal and informal methods to teach other staff members and demonstrates practice based on knowledge of all policies, procedures, nursing standards, and legal issues in areas of specialty. Assist with check-in and monitor the flow of patients through the clinic and facilitates the flow of services to the patient to meet the patient's need. Teach patients and parents about diseases, medications, health promotion practices, and safety measures, and provide anticipatory guidance according to the child's developmental level. Serve as an educator for the patient population and other family members regarding preventative health measures, disease, and disease management in a clear manner. Independent nursing care will be expected, including administering immunizations, IV therapy, and accessing mediports and Hickman catheters. Responsibilities of this position include nursing support, technical assistance, assisting physicians with some procedures, and the activities of maintaining a safe environment, care standards, and inventory control.
Currently, hours of clinic operation are from 8:00 a.m. to 5:00 p.m., with a flexible scheduled late day to accommodate patient care, Monday through Friday.

Minimum Education/Experience:
Graduation from state-accredited school of registered nursing and two years of nursing experience, with at least one year in the assigned specialty area. Must have a current license to practice as a Registered Nurse in North Carolina by the North Carolina Board of Nursing. All degrees must be received from appropriately accredited institutions.
