Job description
COVERAGE REQUIREMENTS Family Medicine (FM/IM)
- Start on February 19 th - End date around August 17 th
- Shift & Call Details: M-F, 8am-5pm
- Credentialing: 2-3 weeks
POSITION DETAILS Family Medicine (FM/IM) - Settings: All - Inclusive Care for the Elderly (PACE)
- Patient Volume: 4-6
- Required Procedures: provide primary care and acute illness evaluation and treatment for participants in PACE program environment. Coordinate with the Interdisciplinary Team (IDT) to develop a comprehensive care plan for each participant.
- The IDT is composed of the following members: • Primary Care Physician; • Registered Nurse; • Master's Level Social Worker; • Physical Therapist; • Occupational Therapist; • Recreational Therapist or Activity Coordinator; • Dietitian; • PACE Center Manager; • Home Care Coordinator; • Personal Care Attendant or his or her representative; • Driver or his or her representative

- Client Req: Board Certification - Internal Medicine or Family Practice, State CDS Certification and DEA Registration
