The ideal candidate will be results-driven, team-and detail-oriented professional who is comfortable working on multiple tasks in a deadline-oriented environment. As a Complex Care CNA, you will provide clinical mental health personal support services, housekeeping and laundry services, food shopping, and transportation in coordination with multi-disciplinary team for clients with complex care needs.
ESSENTIAL RESPONSIBILITIES/TASKS : - Utilize comprehensive nursing assessments, plans, interventions and evaluations in direct client care.
- Monitor medications, teach clients effective techniques for managing medications and assist clients in adhering to prescribed regimens
- Monitor signs and symptoms of psychiatric functioning. Consult with Registered Nurse other treatment staff
- Take vital signs and monitor patients for bruises, other injuries/wounds
- Collect information about conditions and treatment plans from caregivers, nurses and doctors
- Assist with meal preparation, serving and monitoring of nutritional status
- Coach and assist with light housekeeping and maintaining hygienic living environment
- Light laundry duties; Maintain/change clean bed linens
- Assist individuals in transferring from bed to commode to chair
- Turn or reposition patients who are bedridden
- Collaborate with other providers, agencies and individuals in the client's network of care
- Coordinate services within the Organization as needed
- Assist clients in developing and strengthening their support network to accomplish goals and objectives
Education/Experience: - Certified Nursing Assistant credentialing and experience required
- Minimum (1) year experience in a mental health setting working with clients with disabilities preferred
- Experience or training in related state and federal assistance programs also preferred
- Basic computer literacy skills are necessary
Certifications: - Rhode Island certified nursing assistant certification required
