We are actively recruiting for a full-time Certified Occupational Therapy Assistant to join our home health care team in Grand Junction, CO. This role focuses on providing care to adult and geriatric patients in their own homes.
Job Description
Assist Occupational Therapists in implementing personalized treatment plans
Help patients with activities of daily living and adaptive techniques
Maintain detailed records of patient care and progress
Follow all state and federal regulations for home health care
Requirements
Current Occupational Therapy Assistant certification in the state of Colorado
Minimum of 1 year of experience, preferably in home health care
Excellent communication and teamwork skills

- Perform ongoing evaluation of patient’s functional status
- Teach and/or retrain patient’s skills in daily activities
- Provide instruction to patient’s family/caregiver on treatment regimen
- Follow treatment program and goals for improved patient function as advised by the Registered Occupational Therapist
- Maintain appropriate clinical records and submit documentation
- Ensure compliance with all state and federal legal and regulatory requirements
- Additional responsibilities as assigned
- Graduate of an occupational therapy assistant education program accredited by ACOTE
- NBCOT certified with current licensure to practice in state of operation
- 1+ year COTA experience
- Home health experience preferred
- Commitment to clinical and documentation excellence
