Job description

We are seeking a skilled and compassionate Nurse Practitioner or Physician Assistant to join our team in the Northwest suburbs of Chicago. In this role, you will provide outpatient primary care services in a supportive setting, focusing on delivering personalized and individualized care to patients. The clinic hours are Monday to Friday, 9:00 am to 5:00 pm, with two Saturdays per month from 9:00 am to 1:30 pm. You will have a low patient census, allowing you to spend quality time with each patient. Additionally, there is an opportunity to round at the local hospital if desired.

Responsibilities:
- Provide high-quality primary care services to patients in the outpatient setting.
- Deliver personalized and individualized care, taking into consideration each patient's unique needs and preferences.
- Manage a low patient census of approximately 10-15 patients per day in the outpatient setting.
- If desired, participate in hospital rounding and provide care for 2-5 inpatients.
- Collaborate with a multidisciplinary team to develop and implement comprehensive treatment plans.
- Utilize electronic medical records (EMR) systems to document patient encounters and manage medical records.
- Adhere to the clinic's policies, procedures, and clinical guidelines.
- Demonstrate a commitment to patient care and continuous learning and professional development.

Requirements:
- Nurse Practitioner or Physician Assistant certification.
- Current Illinois State License as a Nurse Practitioner or Physician Assistant.
- Valid DEA registration.
- Current ACLS certification.
- Prefer experience in primary care, preferably in an outpatient setting. New Grads welcome to apply!
- Strong clinical skills and knowledge of evidence-based practices.
- Ability to work collaboratively in a team-based care model.
