Child and Adolescent Case Management is an essential and unifying factor in the Hampton - Newport News Community Services Board's service delivery system. The Child and Adolescent Case Manager serves as the single point of contact and support for the family and child regardless of the level of treatment. The Child and Adolescent Case Manager provides ongoing assessments of needs, linking and coordination of services and monitoring of services for young people with serious emotional disturbances or substance use disorders. Activities include interagency planning, collaboration and treatment monitoring to assist families in accessing social, educational, vocational and other supports.
Child and Adolescent Case Managers assist clients with their basic physical, financial, social and personal needs. Child and Adolescent case management services for the client include outreach, psychological and need assessments, treatment planning and service provider linkage, monitoring, advocacy, counseling and crisis intervention, daily living support, medication supervision, and similar functions.
Program services offered:
Requirements: Bachelor's degree in human services (such as: Social Work, Psychology, Sociology, or Counseling) with one (1) year experience in case management services for children and adolescents. Valid Driver's License.
The selected candidate must successfully pass a criminal history fingerprint background investigation, DMV record check, Child Registry search, drug screening test and employment reference checks.
