Job description
BAYADA Hospice is hiring a compassionate and dedicated Nurse Practitioner, (NP) to join our team. The coverage area for this position is New Castle County.
This is a per diem position making face to face recertification visits to hospice clients (our visits happen between the hours of 8:30-5 during weekdays).
Prior home care, hospice, or palliative care experience strongly preferred, but not required. As a hospice nurse NP, you will be an integral member of a multi-disciplinary health care team that provides palliative care to clients, affording them the opportunity to receive the medical care required to remain at home.

Responsibilities for a Hospice Nurse Practitioner include :
Making home face to face hospice recertification visits to clients in designated geographic territories.
Monitoring clients' conditions; reporting changes to Clinical or Client Services Manager.
Following up with, executing, and properly documenting doctors' orders.
Performing client assessments as necessary.
Qualifications for an NP include :
A current license as a NP in Delaware.
A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
Prior home care, hospice and/or palliative care experience strongly preferred, but not required.
Solid computer skills; prior experience with electronic medical records (EMR) preferred.
Ability to travel to cases as assigned.
