Position Overview:
Accountable for patient-care outcomes through the assimilation and application of evidence-based information to design, implement, and evaluate patient-care processes and models of care delivery. Promotes creating the best care plan for patients and their families. With the patient in the center, coordinates care between providers involved in the patients’ care. Provides advisement and counsel regarding complicated patient cases.

JOB ACCOUNTABILITIES:
Clinical leadership for patient-care practices and delivery, including the design, coordination, and evaluation of care for individuals, families, groups, and populations
Participation in identification and collection of care outcomes
Accountability for evaluation and improvement of point-of-care outcomes, including the synthesis of data and other evidence to evaluate and achieve optimal outcomes
Risk anticipation for individuals and cohorts of patients
Lateral integration of care for individuals and cohorts of patients
Participates in the design and implementation of evidence-based practice(s)
Team leadership, management and collaboration with other health professional team members
Information management or the use of information systems and technologies to improve healthcare outcomes
Stewardship and leveraging of human, environmental, and material resources; and advocacy for patients, communities, and the health professional team
Implements/enforces adherence to department, affiliate, operating unit standards/policies
Provides input into hiring/termination
Develops schedules
EDUCATION:
CERTIFICATION & LICENSURE:
RN-Registered Nurse of California
BLS-Basic Life Support Health Provider
DEPARTMENT REQUIRED CERTIFICATIONS & LICENSURES
Department, PALS-Pediatric Advanced Life Support
Department, Intensive/Critical Care Unit, ACLS-Advanced Cardiac Life Support
National Institutes of Health Stroke Scale - NIHSS Certificate
TYPICAL EXPERIENCE:
SKILLS AND KNOWLEDGE:
Solid understanding of operations and workflows and how department’s process/workflow impacts other flows or departments.
Knowledge and understanding of applicable local, state, federal and other health care laws, regulations and requirements – e.g., TJC, Medi-Cal, Medicare.
Demonstrated leadership skills, including team building, and coaching/mentoring with the ability to motivate and engage others.
Demonstrated ability to identify and resolve technical and operational issues problems as guided by policies, and procedures, or guidance from leadership.
Ability to build and establish effective working partnership with employees, peers and/or leaders to achieve business objectives.
Understands age specific needs in providing care to patient population served.
Demonstrates ability to handle confidential and sensitive issues.
Verbal and written communication and interpersonal skills.

Pay Range is $79.69 to $104.37 / hour
The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs. Base pay is just one piece of the total rewards program offered by Sutter Health. Eligible roles also qualify for a comprehensive benefits package.
