Job description
Hospice Admissions RN
The
Hospice Admissions Registered Nurse is responsible for receiving and processing referrals to the Hospice program. Explaining the Hospice philosophy and program; obtains necessary medical information; obtains certification of the terminal illness (verbal and written) from the primary physician and coordinates screening/admission visits.
Minimum Requirements: - Current and valid NY State license to practice as a Registered Nurse
- 1 - 2 years of post-licensure experience in an acute care setting
- Good computer and Electronic Medical Records skills.
- Professional and compassionate communication skills
- BSN and prior experience in home care or hospice preferred
The pay for this position ranges from $27 - 55 per hour.
This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate's qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
