The Nurse Navigator for Adolescent and Young Adult (AYA) Hematology/Oncology is an advocate, educator, and coordinator of care for cancer patients ages 12-39 who have unique needs that differ from older and younger cancer patients at Hackensack University Medical Center (HUMC).
A day in the life of a Nurse Navigator for the Adolescent and Young Adult Hematology/Oncology may include the following responsibilities:
- Directly interacts with AYA hematology and oncology patients and their families with the goal of providing support that is tailored for the needs of the AYA patient
- C oordinates care between services offered at the John Theurer Cancer Center (JTCC) and/or the Children's Cancer Institute (CCI).
- Assists patients/families and clinicians in identifying appropriate AYA clinical trials and facilitating enrollment
- Identifies patients at risk for infertility due to disease or proposed treatment
- Collaborates with the Fertility Preservation team to discuss infertility risk and possible preservation options available
- Refers for psychosocial support as needed and collaborates with social workers and other support staff
- Identifies AYA patients for the survivorship clinics and helps to schedule an initial evaluation
- Enters patient data into RedCap database for AYA Oncology
- Participates in/attends unit, departmental, hospital meetings, committees, councils, task forces, and multidisciplinary rounds, as needed
- Other duties and/or projects as assigned
- Adheres to HMH Organizational competencies and standards of behavior
- Graduate of an accredited program in nursing
- NJ State Professional Registered Nurse License required
- RN with at least 5 years of clinical experience in a relevant clinical specialty (pediatric hematology/oncology, medical hematology/oncology)
- Strong organizational, time management, critical thinking skills
- Ability to work independently and also collaborate with JTCC and CCI staff.
- Excellent written and verbal communication skills.
- Proficient computer skills that may include but are not limited to Microsoft Office and/or Google Suite platforms. Licenses and Certifications
- Nurse Navigator Certification preferred
